However, for hospitals, developers, and manufacturers of healthcare technology, it is not enough to simply observe that a person is walking better: it is necessary to measure what has changed, how much it has changed, and with what degree of reliability.

Biomechanical analysis makes it possible to translate functional progress into objective data. By studying variables such as walking speed, ground contact times, reaction forces, and symmetry between limbs, it is possible to supplement clinical scales and build a more solid body of evidence regarding the outcomes of an intervention.

A study conducted by NedAMH and the Institute of Biomechanics (IBV) demonstrates how this approach was applied to evaluate a Lokomat rehabilitation program for people with acquired brain injury. Although this was a preliminary study, the project illustrates the value that a motion analysis laboratory can provide and the precautions that should be taken when interpreting the results.

The Challenge of Assessing What Changes After Rehabilitation

Robotic rehabilitation technologies are designed to facilitate repetitive, guided movements that can be adapted to the patient’s abilities. Their potential clinical value is significant, but evaluating outcomes poses a challenge: the improvement perceived by the therapeutic team or by the patient is not always accurately reflected by a single scale.

To demonstrate the effect of a product or therapeutic protocol, it is advisable to combine clinical indicators with instrumental measurements. The former help describe functional level and autonomy; the latter make it possible to detect specific variations in movement patterns that might go unnoticed in an observational assessment.

This need is particularly important during the development and validation of medical devices. Defining measurable variables from the outset helps justify the intended use, compare configurations, guide design improvements, and prepare for future studies using a reproducible methodology.

Gait retraining using the Lokomat

The study examined the progress of individuals with acquired brain injury who participated in a gait rehabilitation program. The intervention combined sessions using the Lokomat robotic system with conventional physical therapy; therefore, the results reflect the entire program and do not allow the change to be attributed exclusively to the robot.

Eight patients were assessed using clinical scales before and after treatment. Four of them also underwent an instrumental gait analysis, which allowed for a comparison of biomechanical parameters at both time points. This difference between the clinical and instrumental data is essential for interpreting the scope of the conclusions.

The combination of clinical scales and biomechanical analysis

Clinical evaluation provides a comprehensive overview of functional ability. Biomechanical analysis adds quantitative information about how movement is performed. Together, they allow us to answer different yet complementary questions: whether the patient is making progress, in what specific areas, and whether the observed pattern is moving toward a more efficient or stable gait.

The study examined parameters related to speed, stance phases, braking and propulsion forces, symmetry, and balance. The protocol also included analyses with and without shoes, a relevant methodological decision because measurement conditions can affect the results.

For a pre- and post-comparison to be valid, conditions must remain stable: instructions, instrumentation, calibration, required speed, technical aids, and processing criteria. Standardization reduces non-treatment-related variability and makes it easier to interpret the observed changes.

Biomechanical Evaluation Results

The instrumental assessment revealed favorable changes in several indicators. The maximum reported change was 13% in indices related to walking speed without shoes. Changes were also observed in support parameters and reaction forces.

These data should be interpreted with caution. The reported statistical value, p = 0.06, indicates a favorable trend but does not meet the conventional significance threshold of p < 0.05. Therefore, the study provides an indication of possible improvement that may guide future research, but it is not definitive proof of efficacy.

This distinction is important in technical and regulatory communication. A trend may be clinically interesting and useful for designing a subsequent study, but it should not be presented as a definitive conclusion when the sample size and study design do not support it.

Limitations That Affect Interpretation

Appearance

Interpretation

Sample size

Instrumental analysis was completed in four patients; therefore, the generalizability of the findings is limited.

Study Design

No control group was described against which to compare the observed course of the disease.

Combined intervention

Lokomat was used in conjunction with conventional physical therapy; the effect of each component cannot be isolated.

Statistical Significance

The p-value of 0.06 indicates a trend, not statistical significance based on the standard threshold of 0.05.

Clinical Heterogeneity

Patients’ characteristics and the extent of their condition may influence their response to treatment.

Recognizing these limitations does not diminish the value of the project. On the contrary, it allows it to be used as an exploratory study to identify sensitive variables, estimate variability, and improve the design of future research.

From Clinical Perception to Quantifiable Progress

A well-designed instrumental assessment makes it possible to move beyond general statements such as “walks better” to concrete results: increased speed, changes in weight distribution, improved propulsion, or reduced asymmetry. This traceability is useful for both healthcare professionals and those who design and validate technological solutions.

It also facilitates communication among different professional groups. Engineers, clinicians, regulatory specialists, and product managers can work with common metrics and better understand the relationship between the device’s technical performance and the user’s functional performance.

Lessons Learned in Developing Rehabilitation Technology

There are always new insights that expand our understanding of these types of processes. Thanks to them, both the methodology and the results are improved, leading to increased quality and scalability.

Define the outcomes before beginning the study

Primary and secondary indicators must be established in advance and linked to the hypothesis, the intended use, and the target population. Selecting too many variables without a clear priority increases the risk of drawing ambiguous conclusions.

Establish reproducible protocols

Repeatability of test conditions is key to comparing time points, sites, or product versions. The protocol must document the instructions, instrumentation, data processing, and any assistance used.

Combining Different Sources of Evidence

Clinical scales, biomechanics, patient-reported outcomes, and usage data address different questions. Integrating them provides a better understanding of the intervention’s effect and helps identify significant discrepancies.

Select an appropriate sample

The sample size must be justified based on the expected effect and the variability of the variables. Characterizing the participants helps determine to which profiles the results might apply.

Interpret the changes based on the intended use

A statistical difference does not always imply clinical significance, and a trend observed in a pilot study can be valuable for planning the next phase. The interpretation should take into account magnitude, precision, safety, and the context of use.

Motion Analysis to Validate Rehabilitation Products

Motion analysis laboratories can support various stages in the development of robots, exoskeletons, and assistive devices. These include user characterization, comparison of configurations, performance verification, usability evaluation, and the generation of clinical evidence.

IBV combines biomechanical knowledge, evaluation methodologies, and expertise in human-product interaction. This approach helps transform a clinical need into a measurable protocol and yields results that are useful for technical and regulatory decision-making.

To learn about the available capabilities, please visit the IBV Human Movement Laboratory.

From the Preliminary Case to a Validation Strategy

The NedAMH/IBV study shows that biomechanical analysis can detect specific changes following a robotic rehabilitation program and complement the information provided by clinical scales. It also serves as a reminder that the strength of the evidence depends on the study design, the sample, and the responsible interpretation of the data.

For manufacturers and developers, the main lesson is methodological: evaluation must be planned alongside the product. Defining from the outset what outcome you want to demonstrate, in which population, and using which indicators helps reduce uncertainty and guide development toward more robust validation.

Do you need to evaluate a rehabilitation technology?

At IBV, we help you define the protocol, select biomechanical indicators, and generate objective evidence regarding the interaction between the user and the product.

Contact the Med-Lab IBV team.

The incorporation of robots and exoskeletons into neurorehabilitation programs opens up new possibilities for intensifying gait training.
